  • First call our office for lab orders. Once we receive your results, you will receive a call to schedule your first appointment!
  • If you are currently on a testosterone prescription and are looking to change providers, please fax last lab results alone with last note from your previous provider for continuation of care.

  • Medical Marijuana New Patient Evaluation: $250+ 3% transaction fee
  • Recertification for Medical Marijuana Card for established patients of Alternative Health: $230+3% transaction fee

***Recertifications are valid for 2 years**

Medical marijuana patients will be required to pay at the time of scheduling for your appointment.  

If you need to cancel your appointment, it can be rescheduled free of charge. However, we do not offer refunds.
